MAKING IT ADD UP: REPORTING YEAR 1 OF OUR OUTREACH SERVICES

INTRODUCTION

In the spring of 2024, the DESK expanded its services to cater to the needs of unhoused adults in New Haven by introducing an outreach program. This program aims to provide services directly to individuals living on the streets, focusing on connecting them with essential services, medical support, benefits, and housing. The approach is centered on building trust and comfort with the DESK staff, with initial interactions happening outside the office setting. The report highlights key metrics for Outreach performance evaluation, including the number of individuals connected, service frequency, and progress towards securing housing. The analysis provides insights into the impact of DESK's Outreach initiatives in its inaugural year.

THE REACH

Over the course of a year, 237 individuals engaged with the Client Engagement Specialist or DRC staff for case management assistance, spending an average of 164 minutes per month working closely with staff members on-site. The report delves into demographic characteristics, revealing that a significant portion of clients are aged 56 and older. These insights shed light on the level of engagement and support provided to unhoused adults in the community.

THE DISTRIBUTION

The report examines the services and activities offered to Outreach clients, showcasing distribution counts and the frequency of service combinations. By analyzing these patterns, DESK can better understand the needs and preferences of the clientele, ultimately improving the effectiveness of its outreach efforts. Additionally, the data indicates a positive outcome, with 9% of clients securing private housing over the past year, underscoring the program's impact on facilitating stable living arrangements for vulnerable individuals.
